View Presentation

Many nonclinical tasks can be eliminated by implementing a clinical communication and collaboration (CC&C) solution, empowering nurses to deliver faster, more attentive patient care. This session uses six case studies to demonstrate an outcomes-driven approach to nurse communication that has lowered readmission rates by 32%, decreased length of stay by 3.6 hours, increased transfer center efficiency, and more. Topics covered:

  • Discover opportunities in the care delivery process to benefit from improved communication such as care team collaboration, care transitions, and post-discharge recovery.
  • Learn how six organizations improved care quality and safety while also generating a positive return on investment.
  • Pinpoint strategies to assess your current communication challenges and formulate goals to address your specific needs.

Bon Secours Mercy Health drove a 34% decrease in opioid overdose deaths through a multi-pronged approach including rapid referral and care coordination with community-based drug rehabilitation centers (private, church, university, and VA-based facilities). The team successfully reduced the delay in program acceptance from 4-8 weeks to several minutes, resulting in a 50% increase in the number of patients in treatment. Topics covered:

  • Review Bon Secours Mercy Health’s approach to partnering with local drug rehabilitation treatment centers.
  • Discuss the use of technical resources to drive communication and collaboration.
  • Identify strategies to replicate the program’s success in decreasing overdose ER visits and deaths while increasing the number of patients in treatment.
