Apple iPhone 3GS Promotion

 

  1. PROGRAM DESCRIPTION:  Promotion begins at 12:01 a.m. (EDST) on September 8, 2010, and ends at 11:59 p.m. (EDST) on October 21, 2010. Promotion is limited medical doctors in private practice who become PerfectServe subscribers during the promotion period, and cannot be combined with any other PerfectServe offer or discount. This introductory offer applies only to new PerfectServe Practice accounts. Limit 3 promotional items per practice.

  2. HOW TO QUALIFY:  To qualify for the promotion, your practice must begin forwarding or referring after-hour calls to PerfectServe between September 8, 2010, and October 21, 2010, and must agree to continue subscribing to PerfectServe at the conclusion of your 30-day risk-free trial. Your practice must pay its first invoice to qualify.

  3. PROMOTIONAL ITEM:  Promotional Item is gift card for an Apple iPhone 3GS 16GB. Approximate retail value of promotional item is $199.00 ($199 Apple gift card can be substituted). Promotional items are not redeemable for cash, and no barter, transfer or assignment is permitted. Limit 3 promotional items per practice. PerfectServe reserves the right to substitute an Apple iPhone of comparable or greater value if the Apple iPhone 3GS is unavailable. All taxes related to the acceptance of the promotional item, including sales and income taxes, are the sole responsibility of the recipient. Any and all guarantees and warranties are subject to the manufacturer’s terms and conditions, and recipient agrees to look solely to such manufacturer for any such warranty or claim.

  4. ACCEPTANCE:  PerfectServe will notify qualifying practices via U.S. Mail within 14 days following payment of their first invoice. Each physician in the practice must complete a Promotion Acceptance Form and return it to PerfectServe via Facsimile or U.S. Mail. Allow 6-8 weeks after return of Promotion Acceptance Form for delivery. Acceptance of promotional item constitutes permission for PerfectServe to use recipient’s name for advertising and promotional purposes without additional compensation unless prohibited by law. By accepting promotional item, recipients release and hold harmless PerfectServe and its affiliates, directors, officers, premium suppliers, employees and agents from any and all liability or any injuries, loss or damage of any kind arising from or in connection with this promotion or acceptance or use of any promotional item received.

  1. PROGRAM DESCRIPTION: No purchase necessary to enter or win. Purchase will not enhance chances of winning a prize. Promotion begins at 12:01 a.m. (EST) on October 1, 2009, and ends at 11:59 p.m. (EST) on November 18, 2009. Mailed entries must be received by Sponsor on or before November 18, 2009. Promotion is limited to the first one hundred (100) entries received online, by telephone, fax or mail. This introductory offer applies only to new PerfectServe Practice accounts. Winners will be selected at random from among the first one hundred (100) eligible entries received. Promotion notices will be e-mailed only to medical doctors in private practice. Only those individuals to whom original promotion notices were mailed will be eligible to enter. VOID WHERE PROHIBITED BY LAW.

  2. HOW TO ENTER: By telephone - call Sponsor at 1-877-844-7728 and speak to a sales consultant, or leave a voice message including your name, telephone number and mailing address. By fax — complete the entry form attached to the promotion notice, and fax it to 1-877-844-7728. By mail — detach and complete the pre-addressed, postage-paid entry form attached to the promotion notice, and drop it in the mail. By online — go to www.perfectserve.com and complete online promotion form. Limit one entry per person. Sponsor assumes no responsibility for late, illegible or misdirected entries.

  3. WINNER SELECTION: Winners will be selected at random from among the first one hundred (100) eligible entries received. One (1) winner will be drawn for each ten (10) eligible entries received, up to a maximum of ten (10) winners. All drawings will be conducted on or before November 18, 2009. Odds of winning depend on the number of eligible entries received.

  4. PRIZES: A maximum of ten (10) Prizes will be awarded. Prize is the following: Gift card for Apple iPhone 3G S 16 GB ($199 Apple gift card can be substituted). Approximate Retail Value of Prize: $199.00. Total Approximate Retail Value of all prizes=$1,990.00. Prizes are not redeemable for cash. All taxes related to the acceptance of the prize, including sales and income taxes, are the sole responsibility of the winner. Limit one prize per person. No barter, transfer, or assignment of prize permitted. Sponsor reserves the right to substitute a prize of comparable or greater value if prize is unavailable.   5. ELIGIBILITY: This promotion is open only to those medical doctors in private practice to whom original promotion notices are addressed. This promotion is open to US residents only (excluding residents of the State of Rhode Island and the State of Florida). You must be 18 years of age or older at the time of entry.

  6. GENERAL CONDITIONS: Promotion entrants agree to be bound by the terms of these official rules. All federal, state and local laws and regulations apply. Winners will be notified via e-mail within two (2) weeks of prize drawing(s). Winners will be required to claim their prize within 30 days following the date of issuance of notification. Non-compliance within this time period or return of any prize/prize notification as undeliverable will result in disqualification. Unclaimed prizes will not be re-awarded. Acceptance of prize constitutes permission for Sponsor to use winner's name for advertising and promotional purposes without additional compensation unless prohibited by law. By entering, participants release and hold harmless Sponsor and its affiliates, directors, officers, prize suppliers, employees and agents from any and all liability or any injuries, loss or damage of any kind arising from or in connection with this Promotion or acceptance or use of any prize won.
